I'm a broken record on things like this, but surgery/implantation of stuff should be your last-most resort, because evidence, specifically lack of evidence that invasive stuff produces significant long-term quality of life improvement over exercise, therapies, etc.

Particularly when someone wants to mess with your back, but also feet.

If the doc/specialist/surgeon is so confident, have them give you probabilities on your return to full or very nearly full functionality, and how much of their fee they'll put at risk on those figures. They're asking you to put something at risk - why shouldn't they?
