You can buy athletic cut (8 inch differnce between pants and jacket rather than 6 inch) but the selection of those suits is very limited.

psh. i have an 18-1/2" neck. try finding a dress shirt thats not made for a 400 pound guy with a neck like this. i'm not skinny(i have a gut), but the rest of my frame is like any normal dude. so i have to get my shirts tailored a retarded amount. surprisingly shirts from american eagle fit me great. but they don't really qualify as 'dress' shirts.
