psh. i have an 18-1/2" neck. try finding a dress shirt thats not made for a 400 pound guy with a neck like this. i'm not skinny(i have a gut), but the rest of my frame is like any normal dude. so i have to get my shirts tailored a retarded amount. surprisingly shirts from american eagle fit me great. but they don't really qualify as 'dress' shirts.

Alterations work well enough for me. I really only have 2 or 3 suits.

If you need dress shirts, you might just invest in custom made shirts.

I have the same broad shoulder issue, not just the arm lifting thing, but I'll get shoulder pain after awhile, even with athletic cut. Anything off the rack that fits my shoulders fine = David Byrne giant suit funny looking. Alas, my only recourse is prolly custom made.
