Yeah so they'd likely have a 2018 primary for the special (and I guess Flake could get primaried) and they'd have two separate senate races, one probably Flake v. McCain was re-elected in 2016. The replacement serves until the next state-wide election, which is 2018, and the winner of this special elections serves the remainder of McCain's term (2022).

Everyone doesn't know this, because he's not going to have to resign. Most sick senators just die in office. Honestly can't think of the last senator who was too sick to carry out his duties and resigned. Byrd/Thurmond/Kennedy all just hung on till they kicked it.
McCain might resign if he goes downhill to get another GOP vote in the Senate, but he'll probably die in office.
